I am will to be a chaperon for the Dance I class trip(s) to the Weinberg Center/Kennedy Center during the fall semester of 2001. I will be responsible for supervising students as directed by Tammy Frazier and am aware that I will also be responsible for paying for my ticket to the performances.

Paul Taylor Dance Company, Friday, October 26, 2001, 5:00 PM Departure to Kennedy Center $11.50

Dance Theatre of Harlem, Friday, December 24, 2001, 5:00 PM Departure to Kennedy Center $15.00

Payment is due with the agreement form at least three weeks before each trip. Checks are made payable to Frederick High School.
